Förebyggande och hälsofrämjande åtgärder

Högt blodtryck, hjärtsjukdom (framför allt förmaksflimmer), diabetes, fysisk inaktivitet och rökning är de fem viktigaste riskfaktorerna för stroke. Om alla dessa faktorer kunde elimineras i befolkningen så skulle andelen förstagångsstroke minska med 2/3. Det här är faktorer som individen själv kan påverka i samverkan med primärvården. Rökstopp, kost med frukt, grönsaker och fisk, motion och viktkontroll, samt kontroll av blodtryck, blodsocker, blodfetter och EKG är angelägna åtgärder som samhället behöver stimulera.

De akuta insatserna

När en individ misstänks ha drabbats av stroke ska SOS-Alarm kontaktas omedelbart. En ambulans larmas ut för att hämta patienten och redan i ambulansen påbörjas vården. Vitala funktioner bedöms först varefter man tar ställning till om patienten har stroke och om det i så fall finns kontraindikationer mot akut reperfusion (trombolys eller trombektomi). Om patienten är lämplig för reperfusion kör ambulansen till närmsta akutsjukhus för akut röntgen av hjärnan och om den är invändningsfri inleds propplösande behandling med trombolys.

Insatser på strokeenheten

Alla stroke- och TIA-patienter ska oavsett ålder behandlas, utredas, vårdas och påbörja rehabilitering på särskilda strokeenheter. Dessa finns på akutsjukhusen i regionen. Strokeenhetsvård medför minskad dödlighet, ökat oberoende och en högre andel som kan skrivas ut till eget boende, jämfört med vård på annan enhet. Det är centralt att strokeenhetsvården bedrivs i form av multiprofessionellt teamarbete innefattande medicinsk kompetens, omvårdnadskompetens och rehabiliteringskompetens (sjukgymnast, fysioterapeut, kurator och logoped) samt har tillgång till dietistkompetens och psykologkompetens helst med neuropsykologisk inriktning.

Rehabilitering

Av de som överlever en stroke kommer c:a 1/3 ha ett signifikant handikapp efter ett år. Det är därför viktigt att rehabiliteringsåtgärder (som tidig mobilisering, aktivering och träning) sätts in tidigt efter insjuknandet eftersom det är då som åtgärderna har störst effekt. Efter den akuta perioden på strokeenhet fortsätter rehabiliteringen beroende på handikappets svårighetgrad inom antingen slutenvård, öppenvård eller kommun. I det ideala fallet finns en tydlig och strukturerad modell för utskrivning från strokeenheten där personer med stroke och behov av stöd, vård och rehabilitering möts av kompetens och delaktighet i hela vårdkedjan.

Fortsatta livet

Forskningen kring hur livet går vidare efter första månadernas insatser är tvärvetenskaplig och involverar forskning kring anpassningsprocess, livstillfredställelse och hur coping kan främjas samtidigt som medicinska aspekter på risk för återinsjuknande och kognitiv försämring är viktiga att studera longitudinellt. Anhörigas och närståendes livssituation kartläggs i syfte att finna bättre stödmöjligheter i samverkan med samhälle, kulturella utbud, familj, brukarföreningar och handikapporganisationer. Forskningen inom detta område är stark inom SCV.

Biomedicinsk, medicinteknisk, klinisk och epidemiologisk forskning

SCV möjliggör en mångfacetterad forskning om stroke med utgångspunkt från vårdkedjans alla instanser och ur ett basalt biomedicinsk och medicinkteknisk samt epidemiologisk perspektiv. En del projekt bedrivs i flera områden och överlappar varandra.
