Medical and surgical advances have given young people with long-term congenital conditions or diseases that started during childhood an increased life expectancy. This group of young people is in need of specialist care throughout their lives to increase the possibility of a good and long life. The transfer of young people with long-term medical conditions from pediatric to adult healthcare has received increasing attention in recent decades. This patient group needs to be prepared for the transition to adult healthcare and learn to take over responsibility for their own health, where participation and self-determination are important aspects. To facilitate this transition, transition programs have been developed, however, there has been a lack of concrete evidence of their effects.
What does a transition coordinator do? How do they work, and what tools do they use? What is important to understand about young people who are beginning to take responsibility for their own care?
