Short description

An eHealth platform has been developed and the aim is to test the preventive effects of PCC in patients´ homes on self-efficacy, symptom distress/anxiety and hospitalisation using patients with chronic heart failure and/or chronic obstructive pulmonary disease as example.

The design is a randomized, controlled, parallel, intervention study of PCC versus traditional care alone. Patients will be recruited from primary care, in the intervention group a personal health plan will be co-created between patients and health professionals. The health plan and other communication with professionals will be performed through the EHealth platform. Follow-up will be after 3, 6, 12 and 24 months from inclusion. In addition, health-economic analyses will be performed.
