Developing a palliative care consultation service in primary care: prerequisites and opportunitie

Short description

This project aims to develop a consultation service that can support primary care and municipal home healthcare in matters related to general palliative care. The service may provide guidance on areas such as the early identification of palliative care needs, symptom management, ethical decision-making, care planning and support for family members. By strengthening access to specialist knowledge and advice, the project seeks to enhance the quality and continuity of palliative care across healthcare settings.

The aim is to strengthen competence in general palliative care, contribute to higher quality care and increase the sense of security for both patients and their family members. In the longer term, the project is also expected to reduce pressure on emergency healthcare services.

The project is conducted using a co-design approach in close collaboration with representatives from primary care and municipal home healthcare services. It begins with an assessment of existing needs and resources, followed by the development of a working model tailored to local conditions and designed for practical implementation and evaluation.
