Implementing General Palliative Care in Residential Care Homes for Older People

Short description

The project aims to develop and evaluate a strategy for the early identification of palliative care needs among older people living in residential care homes. The goal is to enable a greater proportion of care to be delivered in the care home, in line with national guidelines, while ensuring that care is tailored to the individual's needs and preferences. The project is conducted in close collaboration with primary care services, care staff, managers and representatives of older people and their families.

Early Palliative Care for Older People in Residential Care Homes: A Strategy to Improve Quality of Life

This research project focuses on developing and evaluating a strategy for the early identification of palliative care needs among older people living in residential care homes. By identifying needs at an earlier stage, care can be better tailored to the individual's preferences, circumstances and priorities.

The project is based on national guidelines for palliative care and forms part of the ongoing implementation of both the National Care Programme for Palliative Care (2023) and the Person-Centred Care Pathway for Palliative Care (2022) at regional and local levels.

The aim is to ensure that more conversations about serious illness take place at an early stage and that medical care plans are developed in ways that enhance residents' sense of security and satisfaction with care. Delivering care directly within the care home can also reduce the need for transportation and visits to hospitals or other healthcare facilities.

The project is carried out in collaboration with general practitioners in primary care, nurses and care staff working in residential care homes. Managers from primary care centres and residential care facilities, together with patient and public representatives, are also involved through a reference group.
