Today’s society presents many challenges and difficulties, both for people living with dementia and for their relatives, as well as for professionals working in health and social care. DEMSAM aims to identify evidence-based solutions that support a functional and sustainable everyday life.
Possible solutions
DEMSAM aims to support Anna in receiving coordinated, person-centred support through a jointly developed care plan involving health care, dental care and social services. This includes coordinating preventive dental care, vision aids, mobility support and adapted transport with home care services and day care.
Digital barriers are reduced through simpler technologies and hands-on support.
A dementia-friendly outdoor environment, both physical and social, reduces stigma and promotes participation. Clear signage, good lighting, safe crossings, seating and access to green spaces enable Anna to move more safely and independently.
Regular communication between stakeholders ensures continuity and early interventions, strengthening Anna’s dignity, independence and social participation. At the same time, her daughter receives structured support as a family carer.
