My PhD project aims to improve the process of quality control of radiation treatment plans and to reduce uncertainties in the treatment of the patient. The project is focused on external radiation therapy with intensity-modulated fields (IMRT).
An individual treatment plan is created for each patient based on the patient's anatomy and pathology. The created treatment plans might therefore vary in complexity. A complex treatment plan means that there is a risk of clinical relevant dose differences between planned and delivered dose distributions that might affect the treatment of the patient.
The work is focused on producing a complexity metric that correlates to the dosimetric differences between the planned and delivered dose distribution. The project is divided into two main parts.
• Develop a method to investigate and evaluate the dosimetric differences between planned and delivered dose distributions. This includes creating fields of varying complexity, choosing appropriate measurement methods and investigate evaluation methods to accurately determine dose differences. The extent of dose difference is used as a measure of the complexity of various treatment plans.
• Develop a model to calculate a complexity metric to determine the complexity of a treatment plan. The model is based on parameters that contribute to an increased risk of dosimetric differences between planned and delivered dose distributions. One example is the treatment planning system's difficulties to calculate dose distributions for very small field openings. Measures related to small and irregular field openings could therefore be used as input when developing a complexity metric.
The idea of the project is to develop a complexity metric to be able to identify and reduce the complexity of the treatment plan and thus reduce uncertainty in the treatment of the patient. A validated complexity metric could be used as a tool for quality assurance of treatment plans and to streamline the quality control process. A complexity metric could also be developed to be implemented already in the planning of treatment plans to reduce the complexity at an early stage.
